To ensure an open pathway during root canal instrumentation, a small, flexible file is intentionally extended beyond the apical foramen – a technique referred to as apical patency filing. Its significance in endodontic therapy remains controversial. Many authors claim that it minimises debris accumulation, improves irrigant penetration, helps prevent apical blockage and enhances the accuracy of electronic working length assessment – all of which contribute to better debridement of the apical third, particularly in complex canal anatomies. However, opponents highlight potential drawbacks, including the risk of compromising the final apical seal, extrusion of debris and irrigants, increased post-operative pain, apical transportation and disruption of the natural apical constriction.
