Antimicrobial susceptibility and clinical outcomes of tosufloxacin for mycoplasma pneumoniae among Japanese children in 2024

Tomohiro Oishi

Background

Mycoplasma pneumoniae is a pathogen that causes community-acquired pneumonia. In 2024, for the first time since 2016, M. pneumoniae infections were epidemic in Japan. Recently, macrolide resistance has emerged among these pathogens. As tosufloxacin (TFLX) is the only antibiotic approved for use in children below the age of 8 years, this study investigated susceptibility and related clinical outcomes of TFLX treatment.

Methods

This multi-center retrospective observational study evaluated 38 children with M. pneumoniae infections. Isolates were collected via nasopharyngeal culture, and TFLX minimum inhibitory concentrations (MICs) were determined. Detailed clinical courses were evaluated for patients treated with TFLX.

Results

The TFLX minimum inhibitory concentration of their isolates ranged from 0.0625 to 0.5 g/mL, that TFLX MIC values remained within a low and favorable range Among seven patients who received TFLX against M. pneumoniae infections, five exhibited defervescence within 48 h of TFLX treatment onset, one was accompanied by COVID-19 infections, and one initiated TFLX on day 10 post-fever onset.

Conclusion

In conclusion, TFLX is currently retains favorable microbiological susceptibility and suggested clinical usefulness.
