Aim

This in-vivo study aims to evaluate the antimicrobial effect of nanobased intracanal medications (ICM) [Triple antibiotic-loaded chitosan nanoparticles (Tachnp) and chlorohexidine (CHX) loaded by silver nanoparticles (CHX-AgNPs)] in comparison to CHX on Enterococcus faecalis count reduction in secondary endodontic infection cases.

Methodology

Thirty patients with single rooted, single canal premolar teeth indicated for retreatment were randomly divided into three groups according to the ICM used; group 1 CHX gel, group 2 (CHX-AgNPs) gel and group 3 Tachnp gel. The first microbial sample (S1) was taken after removal of gutta-percha under complete aseptic conditions, then the second microbial sample (S2) was taken after reinstrumentation using ProTaper Universal rotary system up to F3 or F4 according to canal size using 10 ml of 2.5% NaOCl irrigation solution. Then ICM was applied to each group and during the second visit after 1 week, the third microbial sample (S3) was obtained after removal of ICM. The three samples were cultured then the growing colonies were counted and recorded as colony forming units (CFU/ml).

Results

The third microbial sample (S3) recorded the lowest microbial count in all groups. No statistical significant difference between percentages of reduction of S2 to S1 among the tested groups while there was statistically significant difference between S3 to S2 and between S3 to S1.

Conclusion

ICM have a major role in reduction of E. faecalis where both CHX-AgNPs gel and Tachnp gel showed significant antimicrobial efficacy compared to CHX gel.
