Anomalies of the first branchial cleft in children: clinical picture, diagnosis, options and surgical treatment

Introduction. Developmental anomalies of the first branchial cleft in the form of a fistula or sinus are a rare congenital malformation that can sometimes be acquired as a result of surgical drainage of a branchial cleft cyst. The purpose of this study was to examine the clinical presentation, diagnosis, and surgical treatment of first branchial cleft anomalies in a large group of patients, and to review the relevant literature. Material and methods. A retrospective study was conducted on the basis of maxillofacial surgery at the Russian Children's Clinical Hospital in Moscow from 2018 to 2024. The present study included 69 patients (27 men, 42 women), of which 55 patients had complete data. The average age of the study group was 7.9±0.62 years (from 11 months to 14.5 years). Medical records were reviewed for postoperative complications and recurrence rates. Results. The disease was 1.5 times more common in girls, and the left side predominated among the lesions (42:27). Of the 55 patients, 41 (74.5%) had a fistula as a result of infection of an existing cyst or incomplete previous removal. Of the 31 cases, in 9 (29%) the fistula tract passed under the facial nerve. Despite this close relationship, no patients developed postoperative facial palsy. Of the 55 operations, 52 (94.5%) were successfully performed with complete resection without recurrence in the postoperative period. Only 4 patients with a history of multiple operations had a recurrence, which was successfully eliminated with reoperation. We associated the occurrence of relapses with the fact that 2 of them occurred during the period when operations were performed without optical magnification, and 2 were due to a residual component in the external auditory canal. Conclusion. Anomalies of the first branchial cleft are extremely rare, which makes timely diagnosis and adequate treatment difficult, without which the risk of relapse remains high. The complexity of surgical treatment is associated both with the close connection with the facial nerve and external auditory canal, and with recurrent inflammation leading to scarring and the formation of false passages. The use of fistulography allows you to plan the scope of treatment, and intraoperative instillation of dye to track the fistulous tract and remove it, even in the case of severe scarring and conflict with the facial nerve, even when using the tunnel technique.

Аномалии развития первой жаберной щели (ЖЩ) в виде свища или синуса – редкий врожденный порок развития, который иногда может быть приобретенным в результате хирургического дренирования кисты ЖЩ. Целью настоящего исследования было изучение клинических проявлений, диагностики и хирургического лечения аномалий первой ЖЩ у большой группы пациентов, а также обзор соответствующей литературы. Материал и методы. Проведено ретроспективное исследование на базе отделения челюстно-лицевой хирургии РДКБ (Москва) с 2018 по 2024 г. В настоящее исследование были включены 69 пациентов (27 мужчина, 42 женщины) из которых полные данные были у 55 пациентов. Средний возраст исследуемой группы составил 7,9±0,62 года (от 11 месяцев до 14,5 года). Медицинские записи были проверены на предмет послеоперационных осложнений и частоты рецидивов. Результаты. Заболевание в 1,5 раза чаще встречалось у девочек, а среди поражения преобладала левая ЖЩ (42:27). Из 55 пациентов 41 (74,5%) имел свищ в результате инфицирования существующей кисты или неполного предыдущего удаления. Из 31 случая в 9 (29%) наблюдалось прохождение свищевого тракта под лицевым нервом (ЛН). Несмотря на эту тесную взаимосвязь, ни у одного из пациентов не развился послеоперационный паралич ЛН. Из 55 операций 52 (94,5%) были успешно выполнены с полной резекцией без рецидивов в послеоперационном периоде. Только у 4 пациентов с множественными операциями в анамнезе был рецидив, который был успешно устранен с помощью повторной операции. Возникновение рецидива мы связали с тем, что 2 из них возникли в период, когда операции проводились без оптического увеличения, а 2 – из-за остаточного компонента в наружном слуховом проходе (НСП). Заключение. Аномалии первой ЖЩ встречаются крайне редко, что затрудняет своевременную диагностику и адекватное лечение, без которого риск рецидива остается высоким. Сложность хирургического лечения связана как с тесной связью с ЛН и НСП, так и рецидивирующими воспалениями приводящее к рубцовым процессам и формированию ложных ходов. Использование фистулографии позволяет спланировать объем лечения, а интраоперационная инстилляция красителя – отследить свищевой ход и удалить его даже в случае выраженных рубцовых процессов и конфликте с ЛН даже при использовании туннельной техники.
