Tracheal injuries resulting from blunt neck trauma are rare but serious, requiring urgent surgical intervention. We present the case of an 18-year-old male who sustained blunt trauma to his chest, left forearm, and left lower limb, resulting in tracheal injury at the T1 level. Emergency tracheal reconstructive surgery was performed, and successful anesthetic management was achieved with awake fiber-optic nasal intubation followed by general anesthesia. The patient was later tracheostomized and ventilated electively for 5 days in the intensive care unit. Tracheal injuries following blunt trauma require prompt recognition and management to prevent airway compromise and respiratory distress. A multidisciplinary approach and effective anesthetic management are crucial for optimal outcomes in such cases.
