Adamantinomatous craniopharyngiomas are typically pediatric tumors, rarely encountered in older adults. Their presentation in the elderly is unusual and can easily be misdiagnosed due to clinical and radiological overlap with more common sellar pathologies in this age group. We report a case of a 64-year-old male who presented with visual impairment and persistent headaches. Imaging revealed a cystic suprasellar mass suggestive of a pituitary macroadenoma. Surgical excision followed by histopathological evaluation confirmed the diagnosis of adamantinomatous craniopharyngioma. This case highlights the importance of considering less common etiologies in atypical age groups and reinforces the indispensable role of histology in diagnostic confirmation, especially when clinical assumptions may be misleading.
