An Alternative Approach for Decentered Polymethyl Methacrylate Intraocular Lens Associated with Posterior Capsular Opacification

Abstract

This case report describes an alternative treatment approach for a decentered polymethyl methacrylate (PMMA) intraocular lens (IOL) in a patient with posterior capsular opacification (PCO) following manual small-incision cataract surgery. The patient presented with temporal IOL decentration and dense pearly PCO, 4 years postsurgery. Conventional yttrium aluminum garnet capsulotomy was deemed inappropriate due to the potential for exacerbating visual disturbances. Instead, we performed IOL redialing combined with posterior capsular polishing, yielding a satisfactory outcome. This technique offers a safer alternative to YAG capsulotomy in specific cases with complex presentation.
