Aligning Research with Impact in the Saudi Health Ecosystem: A Viewpoint and Narrative Policy Review

Awad M. Almuklass

Abstract

Despite immense investment in biomedical research, the translation of scientific output into tangible healthcare improvements in Saudi Arabia demands urgent systemic reform. Between January 2017 and March 2023, local institutions produced over 44,000 health-related publications, yet only a fraction can be verified as progressing to clinical or policy application. This viewpoint examines the magnitude of this translation gap. Using the 2021 national R&D expenditure as a baseline and applying a 20% health-R&D proxy, cumulative health R&D investment over a five-year modeled period was estimated at 14.5 billion SAR; however, the lack of mature tracking mechanisms leaves a significant portion of this investment vulnerable to structural waste. While scientific capacity has expanded rapidly under Vision 2030, fragmented funding, limited translational training, and systemic gaps in integrative leadership create direct barriers to real-world adoption. A coordinated national strategy focused on translational science, driven by impact-based funding, is urgently required to ensure Saudi health research consistently translates into national health, innovation, and economic impact.
