We describe a rare coexistence of arteria lusoria (AL) with a right vertebral artery originating from the right common carotid artery. Multimodal imaging confirmed the anatomy. Embryologic interpretation points to persistent carotid–vertebral connections with an aberrant right subclavian artery due to abnormal regression of the right fourth arch. Clinicians should consider this combination in differential diagnosis and during vascular interventions.
