Congenital medial canthal swellings in neonates often mimic dacryocystocele, but may represent vascular lesions requiring medical rather than surgical treatment. We report the case of a 15-day-old neonate who presented with swelling at the junction of the right medial canthus and nasal bridge, initially diagnosed as a dacryocystocele and referred for surgical management. Imaging revealed a vascular lesion, and a diagnosis of infantile hemangioma was made. The neonate was treated successfully with oral propranolol, resulting in a significant reduction in swelling. This case highlights the importance of early imaging to avoid misdiagnosis and unnecessary surgical intervention.
