A Successful Treatment of Alopecia Universalis with Ruxolitinib

Alopecia universalis (AU) is a type of alopecia areata, characterized by total body hair loss and often associated with profound psychosocial distress. Conventional therapies, including intralesional corticosteroids, topical immunomodulators, systemic corticosteroids, and immunosuppressive agents, such as methotrexate and cyclosporine, frequently yield suboptimal or transient responses. We report the case of a 26-year-old male with rapidly progressive AU refractory to multiple standard treatment modalities who achieved complete and sustained hair regrowth following the oral administration of ruxolitinib at a dose of 20 mg twice daily. Remarkable improvement was observed within 12–18 months of therapy, with excellent tolerability and no significant adverse events. This case highlights the therapeutic potential of Janus kinase inhibition in managing refractory AU and underscores ruxolitinib as a promising treatment option capable of restoring both hair growth and quality of life in affected patients.
