A rare case of Sphingomonas paucimobilis keratitis

ABSTRACT

We report a case of 58-years-old male patient who presented with complaints of pain, redness, and diminution of vision in his left eye of 7 days duration. Slit lamp examination revealed central greyish-white corneal infiltration with well-defined margins. On microbiological examination of corneal scrapings, no organism was identified. Patient was empirically started on Fortified cephazolin 5% and tobramycin 1.3% eyedrops but there was no clinical improvement after 48 hours. However, Sphingomonas paucimobilis was identified in culture after 72 hours. The patient was shifted to fortified ceftriaxone 5% and amikacin 2.5% as per the antibiotic sensitivity report. Patient responded well to treatment with resolution of ulcer at 6 weeks. This case highlights the importance of case-based management of corneal ulcer.
