A 32-year-old male presented with features of orbital cellulitis following an attempt to reduce the lacrimal sac swelling by compression, a maneuver he often did to relieve his symptoms. On previous compression maneuvers, the contents used to regurgitate into the nasal cavity, suggesting a diagnosis of an atonic lacrimal sac. The chronic sac inflammation caused nasolacrimal duct blockage, which subsequently led to sac rupture on manual compression and spillover of infective contents into the orbit, leading to orbital cellulitis. The radiological imaging confirmed a dilated drainage system with the ruptured posterolateral wall. This case report highlights a rare complication of atonic sac on self-regurgitation on pressure over lacrimal sac and emphasizes the importance of early intervention in such cases.
