A Novel Piezoelectric Tip for Dorsal Hump Resection: A Randomized Clinical Trial in Primary Open Septorhinoplasty

Objective: To compare the performance of a novel piezoelectric tip with that of a conventional tip for dorsal hump resection in primary open septorhinoplasty, with the hypothesis that the new design would shorten resection time without increasing postoperative edema, ecchymosis, or compromising aesthetic satisfaction.Method: In this randomized clinical trial, adults aged 18–50 years undergoing primary open septorhinoplasty were allocated 1:1 to dorsal hump resection with either a novel piezoelectric tip or a conventional tip. Fifty patients meeting the eligibility criteria were included (25 per group). The primary outcome was hump resection time. Secondary outcomes were periorbital edema and ecchymosis at postoperative day 2, graded using the Kara–Gökalan scale, and patient- and physician-rated nasal shape satisfaction at 1 year using a visual analog scale. Results: The groups were comparable in age and sex distribution. Mean hump resection time was significantly shorter in the novel-tip group than in the conventional-tip group (4.73 ± 0.88 vs 5.79 ± 0.98 minutes, p < 0.001), representing an approximately 18% reduction. Early postoperative edema and ecchymosis scores did not differ significantly between groups. Likewise, patient- and physician-rated nasal shape satisfaction at 1 year were similar. Multivariable analysis confirmed that tip type was not associated with satisfaction outcomes.Conclusion: The novel piezoelectric tip significantly improved procedural efficiency during hump resection without worsening early soft-tissue morbidity or long-term aesthetic satisfaction. These findings support its safe and practical use in primary open septorhinoplasty.
