A Comparative Study of 5% Dextrose versus Triamcinolone in the Carpal Tunnel Syndrome Patients Refractory to Conservative Therapy

Background:

Triamcinolone is used as the most common drug for the hydrodissection of the median nerve at the wrist joint in the carpal tunnel syndrome patients for analgesic effect. To find new drugs to overcome the side effects of steroids, we compare 5% dextrose with triamcinolone.

Objective:

The study was done to evaluate the efficacy of perineural injection of 5% Dextrose versus Triamcinolone for Carpal Tunnel Syndrome patients in terms of pain assessment as per NRS score.

Design:

Randomized, double-blinded, head-to-head comparative trial.

Methods:

A total of 50 patients were included in the study Group A and Group B, with 25 patients in each group, who received one session of perineural injection with 5% dextrose and triamcinolone acetonide, respectively.

Outcome Measure:

The primary objective was to compare the effectiveness of drugs as per the NRS scale, whereas the secondary objective was to compare the effectiveness of drugs as per the Boston Carpal Tunnel Questionnaire severity and functional status, and side effects of drugs.

Results:

There was a decrease in pain intensity in both the groups and there was significant relieve in pain at the end of 6 week approximately 80% of patients have NRS score <6 ( P > 0.05).

Conclusion:

Both 5% dextrose and steroids can be used for hydrodissection of the median nerve at the wrist joint in patients with carpal tunnel syndrome patients refractory to conservative treatment.
