Objectives: To report a case of asymptomatic atrial fibrillation (AF) in which AF suppression and reduced supraventricular ectopic (SVE) burden were achieved with complex Korean medicine (KM) treatment.Case presentation: A male patient in his late 70s with antiarrhythmic-refractory AF received a 7-week KM regimen consisting of acupuncture and sequential herbal treatment with modified Sihogyejigeongang-tang followed by Sayeok-san-based formulas. Treatment response was evaluated using pretreatment and posttreatment 72-hour Holter monitoring. Baseline monitoring showed an AF burden of 8.61% and an SVE burden of 6.19% (18,129 beats). On Day 49, follow-up monitoring demonstrated no detectable AF episodes (AF burden, 0%) and a reduced SVE burden of 0.57% (1,704 beats). Conclusions: Complex KM centered on modified Sayeok-san may represent a viable alternative for managing asymptomatic AF and reducing arrhythmic triggers, although further controlled studies are warranted.
