Perinatal stroke is rare and devastating condition. Any neonate presenting with seizures, altered mental status or sensory-motor deficit should raise a suspicion of stroke. This is a case of a one-day female infant presented with respiratory distress and was kept on high flow nasal cannula (HFNC) support. As her condition improved, she was weaned off from oxygen support. On the third day of life she experienced three episode of convulsions, for which anti-epileptic medications like leviteracetam and phenobarbital were administered. Subsequently MRI image revealed a left middle cerebral artery infarct.
