A case of suprasellar epidermoid cyst causing primary optic atrophy
Avani B Chandran, Virna Shah, Karthik K ManiIntracranial epidermoid cyst rarely manifest in adolescent as isolated visual decline without headache. A 16 year old female presented with painless vision loss in right eye. MRI revealed a non-enhancing suprasellar mass encasing circle of willis, with restricted diffusion on DWI/ADC diagnostic of an epidermoid cyst. She underwent a right pterional craniotomy with near- total excision, leaving a small remnant adherent to the left optic nerve to preserve function. Postoperatively, vision improved with no recurrence. Near-total resection balances neurovascular safety and disease control in young patients.
