A Case of Progressive Paraparesis in a Young Female Due to Spinal Dural Arteriovenous Fistula Treated with Endovascular Therapy

BSTRACT
Spinal dural arteriovenous fistulas (SDAVFs) are rare acquired vascular malformations that frequently present with progressive myelopathy and non-specific neurological symptoms, often resulting in delayed diagnosis. We report a 27-year-old female who presented with progressive paraparesis and sensory disturbances. Magnetic resonance imaging demonstrated extensive intradural extramedullary serpentine flow voids with spinal cord compression and thoracic myelopathy. Spinal digital subtraction angiography confirmed an SDAVF supplied by the anterior spinal artery. The patient underwent successful super selective endovascular embolisation using coils and cyanoacrylate glue, achieving complete obliteration of the fistula. Post-procedure angiography confirmed exclusion of the lesion, and the patient demonstrated significant improvement in motor strength and sensory function without procedure-related complications. Early recognition of SDAVF and timely endovascular treatment are crucial to arrest neurological deterioration and achieve favourable clinical outcomes.
