18F FDG and 68Ga FAPI PET/CT Findings in a Pituitary Abscess Secondary to Tuberculous Meningitis

Pituitary abscess is a rare, life-threatening condition of the sella, accounting for less than 1% of all pituitary lesions. We present the first case documenting ⁶⁸ Ga FAPI uptake in a pituitary abscess, in conjunction with ¹⁸ F FDG PET/CT. While both tracers localized the lesion, ⁶⁸ Ga FAPI PET/CT demonstrated intense avidity against a negligible physiological background in the brain, thereby enhancing the contrast and detectability of this often-minuscule lesion. This superior tumor-to-background contrast highlights the potential of ⁶⁸ Ga FAPI PET/CT for the high-contrast visualization of infectious/inflammatory sellar lesions.
