The aggravation of population aging has brought an increasingly heavy burden of geriatric chronic diseases, most of which are triggered by the disordered aging microenvironment in the body. As a characteristic low-grade, persistent and non-infec-tious immune imbalance during senescence, inffammaging drives the progression of geriatric chronic illnesses including diabetes, degenerative joint disorders, respiratory diseases and cognitive impairment by activating senescence-associated secretory phenotype (SASP), restraining stem cell regeneration and inducing tissue ffbrosis. Conventional symptomatic anti-inffammatory therapies ap-plied in current clinical practice lack targeting speciffcity and fail to reverse tissue damage and functional decline caused by aging. Regenerative targeted intervention can precisely modulate the inffammatory microenvironment of aging and activate endogenous tis-sue regeneration, breaking through the limitations of traditional treatments. This paper reviews the pathogenic mechanisms of age-re-lated chronic inffammation, elaborates the core strengths of regenerative targeted intervention as well as its clinical application value in geriatric chronic diseases, analyzes existing bottlenecks in clinical transformation, and prospects the future of its precise clinical application, so as to provide theoretical evidence for establishing innovative prevention and treatment systems for geriatric chronic diseases.
