Abstract:The gut microbiome plays a key role in regulating host immunity and has recently gained attention for its critical role intumor immune surveillance and immunotherapy. Certain symbiotic bacteria and their metabolites can systematically influence thehost's innate and adaptive anti-tumor immunity through mechanisms like pattern recognition receptor signaling, short-chain fatty ac-ids, and tryptophan metabolites. This can directly impact the effectiveness of immune checkpoint inhibitors (ICIs), chemotherapy, andadoptive cell therapy. While fecal microbiota transplantation has made breakthrough progress in overcoming melanoma resistance,clinical data show that using antibiotics may reduce the effectiveness of ICIs. To provide new insights for microbiome-based preci-sion cancer immunotherapy, this article comprehensively explores the clinical translation potential and challenges of microbiome-tar-geted strategies, such as fecal microbiota transplantation, probiotics, and dietary interventions. It also summarizes the evidence link-ing the gut microbiome with clinical outcomes in immunotherapy.
