Abstract:With deep population aging, adults aged 80 and above are core recipients of geriatric medicine. They frequently have multimorbidity, polypharmacy, impaired organ reserve and great aging heterogeneity. Traditional chronological age cannot accurately refl ect true biological aging, failing to meet stratifi ed evaluation and risk warning needs of precision geriatrics.DNA methylation aging clocks quantify aging progress and biological aging rate through epigenetic modifi cation, acting as stable, repeatable evaluation tools in aging research. Classic clocks such as Horvath and Hannum derive from European and American cohorts. Due to gaps in genetics, lifestyles and disease patterns, these models poorly fi t Chinese seniors over 80, most of whom suff er from multiple illnesses and frailty. They cannot accurately depict local aging rules and hamper prevention of geriatric chronic illnesses and cardiovascular/cerebrovascular risk prediction.Based on clinical demands in geriatrics, this paper reviews relevant global studies, analyzes drawbacks of universal aging clocks for Chinese elderly, and proposes frameworks to build a localized methylation aging clock for domestic octogenarians. It discusses model construction rules and clinical value, focusing on its potential for cardiovascular/cerebrovascular risk stratifi cation, early warning and personalized intervention. This work off sets the poor suitability of foreign aging models, offering theories and references for localized precise aging assessment, tiered cardiovascular prevention and epigenetic precision geriatric development.
