老年住院患者跌倒风险评估与针对性护理干预研究（Assessing Fall Risk and Implementing Targeted Nursing Care for Hospitalized Older Adults）

Abstract：Older inpatients are at high risk of falls due to declining physical function, multiple chronic diseases, complex medication use, and changes in the hospital environment. Falls may cause fractures, soft tissue injury, functional decline, and fear of falling. They also affect nursing safety and hospital quality management. Based on a review of relevant studies, this paper analyzes fall risk assessment tools, major risk factors, and current nursing problems among older inpatients. It then proposes targeted nursing interventions, including dynamic assessment, risk stratification, environmental optimization, medication monitoring, health education, psychological support, and focused rounds for high-risk patients. The study argues that scientific assessment is the foundation of fall prevention, and individualized nursing is the key to reducing falls. Clinical nursing should shift from simple warnings to whole-process risk management to improve patient safety.
