柔性可导航抽吸输尿管通路鞘与智能压力控制鞘治疗老年感染性肾结石的随机对照研究(A Randomized Controlled Study of Flexible Navigable Suction Uret eral Access Sheath versus Intelligent Pressure-controlled Sheath in the Treatment of Infectious Renal Calculi in Elderly Patients)

Abstract:Objective To explore the efficacy and safety of flexible navigable suction ureteral access sheath and intelligent pressure-controlled sheath for the treatment of infectious renal calculi.Methods A total of 98 patients with infectious renal calculi treated in our hospital between January 2024 and June 2025 were enrolled and randomly divided into Group A and Group B via random number table, with 49 cases in each group. Group A was treated with flexible navigable suction ureteral access sheath, while Group B received intelligent ureteroscopic suction sheath integrated with pressure sensors. The stone-free rate (SFR), renal pelvic pressure, operation duration and postoperative complications were observed and compared between the two groups.Results Group A presented a higher stone-free rate and shorter operation duration than Group B, with statistically significant differences (P<0.05). The mean and maximum intraoperative renal pelvic pressure in Group A were markedly lower than those in Group B (P<0.05). The incidence of postoperative fever and systemic inflammatory response syndrome (SIRS) in Group A was lower than that in Group B (P<0.05). Although the incidence of urosepsis was lower in Group A, no significant difference was found between the two groups (P>0.05).Conclusion For flexible ureteroscopy of infectious renal calculi, the flexible navigable suction ureteral access sheath (FANS-UAS) can effectively improve the stone-free rate, shorten the operation time, and reduce infection-related postoperative complications by greatly lowering renal pelvic pressure when compared with the intelligent pressure-controlled sheath.
