Abstract: Objective To explore the internal logic of the participation of primary medical and health care institutions in the implementation of integrated medical and elderly care policies in the digital context, to provide new ideas and empirical evidence for optimizing the implementation of such policies in China, and to promote the high-quality development of China's healthcare sector.Methods A qualitative analysis was adopted. Based on synergetics, a three-dimensional analytical framework of "subjects-resources-services" was established. A typical case study of D Town Health Center was conducted to analyze the pathways for primary medical and health care institutions to participate in the implementation of integrated medical and elderly care policies in the digital context. Results The study found that the process of primary medical and health care institutions participating in the implementation of integrated medical and elderly care policies is essentially a multi-dimensional synergistic system engineering. The participation pathway of D Town Health Center can be summarized as achieving subject synergy through the "integration of two institutions"; achieving resource synergy with the help of digital platforms; and achieving service synergy through integrated services. Conclusion The participation of primary medical and health care institutions in the implementation of integrated medical and elderly care policies should be guided by policies, fully utilize digital technology, and build an integrated service system.
